Medical expenses are calculated at 30 yen per medical point that is designated by Ministry of Health, Labour and Welfare.
We accept Japanese Yen cash, and credit card.

If you are covered by Japanese health insurance, an administration fee will be added to your medical bill.
First visit: ¥3,300 / Return visit: ¥1,100

1. Present your valid photo ID (passport, military ID, residency card, etc.)
2. Present your Japanese health insurance card or individual number card called “My-Number-Card” if you have

4. TRICARE authorization is required for all TRICARE Prime.
5. Please bring a completed questionnaire, however,

   signature on the bottom of it should be done at the reception on the first visit.

7. Civilian clothes are required due to preventive measures of infection.
8. Please comply with all staff instructions during and after your visit.
9. Acts of violence, threat and harassment, etc. are strictly prohibited.
10. For TRICARE Select and Space A, please make full payment upfront, then submit a claim for reimbursement.
11. For HIGH FEVER, please notify us in advance. Entrance for Designated Patients, 1F, is to be used for such patients.

Prescription form expires after 4 days including the issued day. You can receive the prescriptions at the pharmacy 1F, but you need to pay in Japanese yen or credit card.
